This page will hopefully answer your questions about the application, since the base application just dives straight into the action. Don't think you can get by with just reading this page and filling out the application, you should still read everything else!
For the sake of simplicity, most of the time, Mythos and other-aligned characters as a whole will be referred to as "good guys" while "bad guys" refers to the AMO members as a whole. There are some instances where I will also be referencing only those who are "reborn" or only those who are non-Earth-reborn AMO members. This will be clearly marked.

Both applications have been combined to conserve space.

II. Character Application. Basic Stats.

Name/Alias:
This should be your character's full name (for good guys) or their full alias (for bad guys).
For good guys, it is suggested that the name have a relevant meaning, but it doesn't have to be a super obvious pun. Relevance is nice, but it's not necessary. Also, your character's name should reflect where they are from. A student from New Dehli should have a name that is Hindi; a second generation Mexican student will probably have a Spanish name, and students/teachers who are just from America can choose from a wide variety of sources for their name, but there probably shouldn't be any white girls of Polish decent running around with the name "Qu-Xing Katya Motoyama."
Bad guys will generally have names that are either very punny/relevant to their element or are reminiscent of their real name on Mythos. They may also choose to not give a complete name, or possibly have a name that doesn't sound entirely real (A girl named Freddie Mcsteinowitzsky?).

Meaning:
A good resource to find names (and their meanings) is behindthename.com. Simply list the origin and meaning of the name, and then, if you want, add a little blurb about why their parents chose the name. For example:
Juliet - French/English - Of "Romeo and Juliet" fame. Juliet comes from the  French pet name "Juliette" which comes from the name "Julia", the feminine form of Julius--the Roman emporer--which possibly means "downy beard" a reference to Jupiter, who was based on the Greek god Zeus.
Fischer - A common surname of German decent meaning "fisherman".
A Note About Japanese Names: Please don't use a Japanese name unless your character is from Japan, and if they are from Japan, please use a real Japanese name (note: Tenshi Sakura is not a real Japanese name). If you really want to make a Japanese character and are trying to come up with a Japanese name, please contact me, I'll be able to help you.

Gender: male or female. While the ever popular "transformation transexuals" are acceptable, not everyone should be one.

(Apparent) Age: Ages for each alignment are outlined in their respective information sections

Birth Date: (+ astrological sign) This is the real birth date for anyone reborn (even if they are now in AMO) and an assumed birth date for those who weren't reborn on Earth. The astrological sign should at least slightly reflect their personality (it doesn't have to be a carbon copy). There are a number of places on the web that give astrological information.

Blood Type: Blood type is relevant in many societies and is said to predict a person's personality. This is just another helper (like the astrological sign) to give you some ideas for formulating a personality. There are a number of websites with information regarding personality = blood type correspondence.

Birth Place: Not everyone in Columbus is from Columbus. In fact, the majority of students aren't, and a small percentage is even from out of the country. This doesn't mean that every character should be from Japan, though. Japanese students don’t actually make up the majority of Asian exchange students at OSU.
If your character is a non-reborn AMO member, they are obviously from Mythos, but should have made up a place where they come from. It doesn't necessarily have to be a real city (or even a real country, but you're pushing it there).

Occupation:
Depending on their academic standing, your character could be pretty much anything. Undergraduate students can hold any reasonable college job--retail, work study (get paid more, but the money goes toward tuition/living expense), internship (at a local business or in your major department they usually don't pay more than one or two dollars an hour, if at all), anything that doesn't require experience but gives it.
Graduate students, in most cases, aren't allowed to hold real jobs. They usually work as research assistants or teaching assistants (teach a low-level undergraduate class).
Adults can have jobs as faculty (NOT professors), staff, supervisors, tech support, custodians or construction workers at just about any place on campus.
Professors are generally people with their masters or PhDs (there aren't many PhD owners under 37 in the liberal arts fields) and, at the medical school campus, there are a number of Doctors/Dentists/Optometrists/Research Professionals/etc.

Student Involvements: Excluding MYTHOS organization or Alpha Mu Omega, these are any clubs, sororities, frats or organizations your character might belong to along with any position they hold (for example, someone may be an Honors student in a sorority, president of the scrabble club, a member of the aikido club, and a member of an organization like Allies for Diversity).
Not all characters have to be in organizations like this. Most adults can't be, with the exception of faculty, who can head up and/or sponsor student-run official clubs.
There are hundreds of official clubs at OSU, but please don't go completely off the edge making up a weird club for your character to be in, although pretty much anything you can think of is already spoken for.

Academic Standing: Is your character a first year freshman? Or maybe a fifth year senior? Maybe they're a third year but still a sophomore, or a first year with junior standing? That's what this means. Other categories are Graduate Masters, Post-Masters and PhD or Graduated. If your character is something else entirely, please clarify here.

Majors/Minors: What is your character's major or minor? Please be realistic here. OSU has a ton of majors, but I can tell when something is fake. Also, most people don't have more than two majors or one major and one minor. Almost all the time, combinations make some sort of sense. Here are some examples.
Majors: Japanese & Linguistics.
Major: Economics. Minor: Chinese
Majors: Psychology & Philosophy
Major: International Studies. Minors: Russian & Linguistics
Most combinations are main major + language or main major + related field. Most first years, during their first quarter, decide to list themselves as "undecided" or "exploration" and many programs (med, nursing, dental, journalism) have pre-programs, and you aren't automatically accepted into your field.

Schedule: This is where you should detail your character's schedule. You can find the master schedule on the OSU main webpage at: http://adsd1.it.ohio-state.edu/masterschedule/msched.aspx
Almost every class is either 5 hours or 2 hours, with quite a few being 4 hours (psychology courses, mostly). The majority of students take 15 credit hours. Less than 15 means you're not a "full time student". 20 is the average number of hours for students who are behind or who want to get their classes done quickly. If you take more than 20 hours, you are required to be in counseling to manage your time (and make sure you don't commit suicide--literally) and more than 22 hours isn't allowed.
Now that the technical stuff is out of the way, this section should give us a look into your character's interests. Why are they taking this class? Are they into the material or do they have to for their major or general education requirements? Is the class super boring to them? Do they even show up? What're they getting in it?
Once again, be realistic. A freshman isn't going to be taking a 600-level course.
When applying, be sure to make sure you find out what quarter the game is in (on the timeline page) and scheduel classes that occur in that quarter of 2006/2007.

Likes:
Dislikes:
Hobbies:
Please don't come out of left field with these. If your character likes "cheese" and "penguins" they should have a good reason for liking "cheese" and "penguins"--even if it is because they think they're hilarious. Also, you don't need to list a ton of things, just the most important ones. If going to movies is a big part of the character's life, list that. If obsessively checking their buddylist is a hobby, add that. If they hate dogs because they’re allergic or they were attacked when they were young, that's important, but if they hate cacti for no reason, not so much.

III. Character Application. Persona.

Personality:
This section should tell me the most about your character. It should describe why they are who they are; why they like, dislike and do what they do. It should detail their idiosyncrasies and their habits; what makes them happy, sad, angry or frightened. Be sure to also mention their psychological strengths and weaknesses, too. As mentioned, it doesn't have to be a full psychological profile, but don't skimp on the important details.
Two paragraphs is too short, and probably won't earn you an Accept.

Skills:
Any particular strengths (especially strange ones) should be placed here. These can range from "extremely flexible" to "has weak telekenesis". Each of these skills should be explained with at least a few sentences of information. Basically, these are special skills/abilities/traits that manifest themselves when the character ISN'T transformed, so abilities the character has only when they are transformed need not be mentioned here.

Appearance:
This can be done in two ways: in one big lump of paragraphs, or in points with a sentence or two describing each point. The points to cover are as follows:
Hair, Skin, Eyes, Facial Features, Height, Weight, Build, Extra (do they chew their nails or have scars), Fashion Sense, Accessories, Voice, Carriage (how a person walks, acts and holds themself in public and in private).

History:
From birth to now. Be sure to cover all the important events: where they lived as a child, any life-altering experiences, their childhood, their time spent in high school, what made them decide to come to Columbus, how their college life as been, where they're living now, how they're doing financially, etc. etc.
Don't go into past life information here, unless your character is a non-Earthling AMO member (that will be explained in a second) or you feel it's necessary. You'll have to take care of all Mythos information for your character if you're accepted.

For AMO members, you DO need to delve a little into life on Mythos. However, since many AMO members have had their memories altered in some way, and so much time has passed, the histories as they remember them may be completely different from what truly happened. You should definitely start with how they were turned to Rage--don't go into too much detail about time prior to that. Details regarding what happened between the final hours on Mythos and their arrival on Earth are explained in the story and information sections.
Also, AMO members will probably have Earth histories that are completely fabricated--with fake parents and a fake childhood and a lot of fakey fake fakeness. Since AMO members may or may not be entirely sane, and may or may not be completely aquainted with Earth culture, they can fudge a few things and it can possibly sound a bit odd.

Relations:
Immediate relations that directly affected the character. A mom, a dad, siblings, grandparents who raised/helped to raise them, aunts or uncles that are especially important, any best friends they have now, any enemies they have now, roommates, etc.
If the characters listed are NPCs, you need to provide a name and a short description for each (a longer description is necessary if the character is going to actually appear in the timeline). If the relation is a character already in the game, simply provide the character name and how they are related.
With AMO members who weren't reborn on Earth, all "human" relations are obviously fictional, and no description or name is necessary. You just need to clarify whether or not their fake parents are still together or even alive. If they are related in some way to another AMO member, or feel closeness or enmity toward another member, list that member and their relation here.

IMPORTANT!!
Never withold ANY information from the GM. If you are planning an important plot for your character while you are making them, write about it in as many sections as you can. If your character has a secret that they don't want anyone else to know about, mention it and then tell me at the bottom of the application in the Secrets section. Also state whether or not the secret should be kept from the entire playerbase or just keep it from being In-Character knowledge.

IV. Character Application. Magical Girl/Boy Parts.

STATS
Rank/Alignment:
"Bad Guys" have clearly outlined ranks. See the information section.
"Good guys" can either belong to the MYTHOS organization or they fall under the category of "other". Check out the info section.

Name: Sailor/Hoplite (WHATEVER)

Element(s) of Influence: List the elements you're using here, with the primary element being listed first. You don't need to go into detail. For example, if your plans for Artemis include abilities connected with hunting & wilderness primarily, list those, but if you also plan on using signature attacks that have to directly do with virgin/maiden dances, list that as well.

Symbol: The character's symbol should be described here. Provide a picture if you like. Symbols.com is an awesome resource for finding symbols to use.

Challenge: Their challenge phrase. The typical thing to say is "I am Sailor/Hoplite NAME! In the name of NOUN and NOUN I will VERB you!"

UNIFORM
Locket: Describe the locket (in detail) here.

Color Scheme: (primary + secondary; avoid overlap) No two characters should have "blue" as their primary color, but one character can have "blue" as their primary and another can have "blue" as their secondary. Also, be specific, but don't try to put one past me: I know charcoal and gray are different, but trying light pale blue when someone is already using pastel blue is just too similar. You don't even have to just pick one color. For example, Amphitrite's main color is green--but it's fish scales, not just a color. Maybe someone has a white, lace outfit? Or an irridecent outfit?
AMO members will all have dark or faded colors.

Description: Describe the uniform here, specifically noting any alterations to the base, any accessories, and whether or not the footwear is standard or unique (also, describe the footwear if it is unique).
AMO's have a lot of work to do, as their uniforms all have different levels of decay on top of having their own unique touches.

ABILITIES
Transformation: Transformation phrase here. "(NAME) Power! Make Up!" for "good guys" and "(NAME) Enrage! Make up!" for "bad guys".
Description: What happens during the transformation? Is the character wrapped in ribbon or consumed in fire or clouds? What parts of their uniform manifest first and how? Do they have a final pose they strike at the end? What is it?

Attack 1
Description: Describe the attack. How is it executed? What does it look like? What EXACTLY does it do? Does it have a range? Does it hit more than one target? Does it only affect a certain kind of person or only work under certain circumstances? Be sure to outline strengths and limits of the attack. The attack description should total two paragraphs: one for description of what happens and another for strengths/weaknesses/etc.

Other Attacks: As mentioned in the information section, "bad guys" members, depending on rank, can have up to 4 signature attacks. Attacks after the first one CAN have the characters name in the attack phrase ( i.e. Venus Love Me Chain) but usually don't (Burning Mandala), if a character has four signature attacks, the fourth one should definitely use the character's name (i.e. Jupiter Oak Evolution, Mercury Aqua Rhapsody).
"Good guys" will quickly develop new signature attacks (based on your game participation), on top of having combo attacks with other characters. So, start brainstorming on new attacks once you're accepted.
All new attacks, post-acceptance, will have to be submitted to me for review so I can OK them.

Special Skills, Traits, Innate Magical Abilities or Mythite Magic:
Hoplites will rely more on these than Sailor Soldiers will. This does not mean that one gender has more than the other. Someone can have a ton of useless skills traits and abilities and never use them, and someone else can have one extremely useful skill that they use all the time.
Special Skills - These are skills that a character possesses that are out of the ordinary. Super agility. Very acute senses. Especially handy with knives. Etc. Not everyone will have a special skill, but they aren't uncommon. They are, by far, the most common of all the extra abilities. Each trait should be listed and described in detail.
Traits - Traits are characteristics that are not typically human--maybe they have webbed feet or wings or their skin is gray or their eyes become catlike. These are mostly passive things that change the appearance or nature of the person. Not everyone will have these, but they add a nice touch when you're trying to be relevant. Each trait should be listed here and described in detail, as well as mentioned in the uniform description if they are part of physical appearance.
Innate Magical Abilities - Are similar to Special Skills but are blatantly magical: possessing second sight, telekenesis, teleportation, floating, shooting rainbows, stuff like that.
Mythite Magic - Exclusive only to The Remnant rank of the AMO (and Sailor Geras), Mythite Magic is as it sounds--magical spells that those born on Mythos possess because of their family line or occupation. Maybe, in their past life, your character came from a long line of court magicians who knew simple conjuration spells--perhaps, because of this, they know a few spells. Maybe your character was a priest or priestess who could use magic to heal the sick, they probably still have a few of those spells still memorized.
In the case of SailorGeras, who was born a powerful enchanter, she has magic that roughly equates to Jedi Mind Tricks.
Not all members of The Remnant will have Mythite Magic, as not all people on Mythos were magicians, witches or wizards.

Pandora: Pandoras are covered in the AMO information section

Weapon: Start out by naming it. Then say what it is. Then describe it. For example:
The Star Destroyer. A Giant Laser Cannon. A giant metal laser cannon with lots of shiny knobs and buttons that shoots giant lasers. SailorWhatever can't lug this thing around, and instead only calls upon it for her signature attacks.
It should probably be more detailed and less lame than that, but you get the idea.
If the weapons aren't of much significance, you can just say what they are. For example, Amphitrite uses nets for her attacks. She doesn't name them, so they're just "fishing nets".

If you need any more help, see one of the accepted character profiles to get an idea of what you're supposed to be doing.
